Medical Release Form   Player Registration Form for 2010/2011 season Adobe® Acrobat® Reader is required to view the Registration/Player Registration Form 2010-2011.pdfonline documents. It can be downloaded free from Adobe's site.

To complete player registration for the 2010-2011 soccer season, the following documents are needed:

	Birth Certificate		$125 for Registration Fees
	Player Registration Form		Medical Release Form

Complete the forms and send them along with a copy of your child's birth certificate and registration fees to:
