Michigan State Premier Soccer Program (MSPSP)  Timeline for PYSA Sponsored Teams

To complete player registration for the 2007-2008 soccer season, the following documents are needed:

	Birth Certificate		$100 for Registration Fees
	Player Registration Form		Medical Release Form

Complete the forms and send them along with a copy of your child's birth certificate and registration fees to:
