*Note: We are utilizing GotSport.com this year for tournament registration, all teams must use their current team account to register, or create a new one, as needed.
* All Coach Check-in documents will be uploaded via Sports Form. Once Registration is accepted, you will receive an email. 
*Teams may be accepted as we fill brackets on a rolling acceptance basis. If we are over capacity before the deadline, we will send out decline or waitlisted emails by mid April.
*This event may reach capacity prior to the deadline.

• PYSA reserves the right to combine divisions as deemed necessary. 
• Typical Tournament Format; 1 division per age group, pool play with quarter-final (as necessary), semi-final (as necessary), and final. 
• Registration Deadline is March 1, 2023, but divisions are expected to be filled earlier. Teams may be accepted as we fill divisions on a rolling acceptance basis. 
• All payments must be entered or check received by the deadline to be considered for acceptance. Your card will be charged or check cashed once accepted. 
• Once accepted, you will receive a document upload request from SportsForms. Documents will NOT be uploaded via Gotsport. 
• DO NOT APPLY IF YOU ARE UNSURE ABOUT ATTENDANCE. EMAIL [email protected] with questions. PLACEHOLDERS WILL BE MOVED TO THE WAITLIST.
• This is not a stay-to-play tournament, please review our Petoskey Area Visitors Bureau accommodations page for availability.

COACH ONLINE CHECK-IN

MANDATORY

All COACHES must check in online prior to the tournament.  
DO NOT EMAIL THE DOCUMENTS. THEY MUST BE UPLOADED AND VERIFIED THROUGH SPORTSFORMS. Teams will receive an instruction email in mid April. 
Upon acceptance you will receive an email from SportsForms for upload and verification. Be prepared to upload the following items. 

a. Official League Team Roster (current season Fall 2023-Spring 2024).
b. Valid Risk Management on all team officials present
c. Valid Heads Up Concussion Certificate on all team officials present

d. Required Liability Waiver (parents signed)
e. Team Official Waiver (signed)
f.  Guest Player Approval Form (if applicable)
g. Travel Permission (if applicable)
