You must mail in a signed Lompoc AYSO registration agreement form, a signed parent/athlete concussion awareness form, the completed on-line registration, and proof of payment to Lompoc AYSO at PO Box 1211, Lompoc CA 93438 OR bring to Walk-in Registration
Walk-in Registration will be at the DeWees Community Center 1120 W. Ocean Avenue, Lompoc
